“Human in the loop” means that a person reviews and approves at the decisive points while an AI system automates the rest: the AI does the groundwork, the human keeps control. It is not the opposite of automation but its precondition. Only if you decide where decisions are made without a person and where they are not can you extend automation safely.

Think of an assembly line where the machine does most of the work and an inspector checks the result at the end. Good automation works the same way: the human does not check every step, only the points where a mistake gets expensive.

What does “human in the loop” actually mean?

The term comes from engineering and AI research: a system does not decide alone but hands over to a person at a defined point. In practice the AI reads the enquiry, drafts a response or proposes an action, and waits until someone reviews and approves it.

The important distinction: human in the loop is not a full review. If you check every email by hand, automation saves you hardly any time. The approval point sits where the decision becomes binding.

Why good automation still asks you

AI systems produce answers that sound plausible and are occasionally wrong. This is called “hallucination”: the text reads convincingly, but the number behind it is off. In an invoice or a contract draft, that can quickly become expensive.

The AI also often lacks the context you know from day to day: long-standing customers, special agreements, internal arrangements. An approval step catches exactly that. The tier depends on the decision, not on the tool.

The rule is simple: the more binding the decision, the stricter the approval. If you also check every answer in the low tier, you only create a new queue.

What is an audit trail and why does it matter?

An audit trail is the complete record of every automated step: what the AI suggested and when, which data went in, who changed the draft and when it was approved.

It matters for three reasons:

  • Traceability: after an error you can reconstruct the path.
  • Accountability: it is clear who decided.
  • Improvement: clusters of errors show up in the log, so you can adjust the rules in a targeted way.

Without an audit trail, automation quickly becomes flying blind.

How do I set up human in the loop in my business?

Four steps are enough to start:

  1. Break the process down: which decisions are routine, which are binding?
  2. Define the approval points: where does the AI stop, who may approve, and how quickly?
  3. Name the responsibilities: who checks what, including during holidays and sickness.
  4. Turn on logging: every AI decision is recorded with timestamp, inputs and approval.

Checklist: Is your process ready for human in the loop?

  • The process is documented in writing, exceptions included
  • Risk tiers (low, medium, high) are set per decision
  • Each approval point has a named person and a deputy
  • The AI knows when to ask and when to continue automatically
  • The audit trail records input, suggestion, change and approval
  • Open items are reviewed at fixed times
  • For errors, it is clear who corrects them and who adjusts the rules

Conclusion: automate the routine, keep the decision

Human in the loop is the difference between “the machine does it” and “the machine does it, and we check where it counts”. What matters is the clear definition: which tier, which person, which approval point and what the audit trail records. That keeps automation a gain in time rather than a risk to your customers’ trust.
